I am in receipt of the settlement and release agreement pursuant to the conversation held on [INSERT DATE] where we settled the claim referenced above. I’ve taken an opportunity to review the settlement and agreement, and upon my review I’ve discovered the following errors:

- Incorrect Party Names
- Incorrect Releasors
- Incorrect Settlement Amount
- Factual Mistakes
I respectfully request the settlement and release agreement be amended to accurately reflect our agreement of the settlement.
